The Serb battled with the sweltering heat and humidity and was beaten by Argentina's Thiago Tirante in three sets. Djokovic, the No. 3 seed in Ohio, hadn't been seen since Wimbledon but ran away with the first set 6-2 as he faced Tirante in the round of 64. However, as the temperature on court reached a roasting 35°C, the seemingly ageless 39-year-old began to struggle. A gruelling 18-minute third game in the second set was when Djokovic reached breaking point. He held serve for 2-1 lead, but after winning the game, he collapsed to his knees and appeared to vomit into a towel. Djokovic then called for a medical timeout and his blood pressure was taken. He was assessed by doctors whilst being given ice towels to cool off. Djokovic managed to play on, but ultimately found victory was out of his reach, losing 6-2, 4-6, 4-6 to the world No. 50. Speaking after the match, he revealed that it was not solely the temperature that contributed to his struggles but that an existing health issue exacerbated the situation. "First of all, congratulations to my opponent. Honestly, this was not a pleasant match for me at all. That's how I felt. It's not the first time, I guess it won't be the last, but whatever," he said. "The weather conditions were not decisive... It's simply a health problem that I have. It's been bothering me for a few years. "I have a lot of problems, especially when the humidity and heat are high. I was aware of the weather conditions, but other things also affect it - nervousness and everything that goes with the match. All of that makes the situation even worse, that's what happened to me." He added: "Unfortunately, at this point, it's more likely that I won't be playing in Cincinnati next year. We'll see what the future holds." The 39-year-old has been crowned champion three times at the ATP Masters 1000 event. He first won it in 2018 by defeating Roger Federer in straight sets. Two years later, he beat Milos Raonic in the final, becoming the first player to achieve a Double Career Golden Masters - winning every Masters 1000 tournament at least twice. In 2023, he defeated Carlos Alcaraz 5-7, 7-6(7), 7-6(4). Djokovic saved a match point in an intense three-hour 49-minute epic, which remains the longest best-of-three-set final in ATP Tour history.
